The most common deficiencies our surveyors see for Chapter 12 “Pathology and Medical Laboratory Services” of the AAAHC Accreditation Handbook for Ambulatory Health Care:
The facility’s CLIA certificate must be up-to-date.
All test results must be documented in the patient chart.
Clinical staff must have annual competencies for all tests administered.
Blood glucose testing must use a multi-person glucometer and staff must follow the cleaning requirements of the manufacturer.
Urine pregnancy test kits must not be past the expiration date.
Specimen logs should have the patient name or number, location of tissue taken, date of service, name of the lab, date the pathology report was returned, and date the provider reviewed report.
